Relapse Prevention (RP) is an evidence-based cognitive-behavioral approach to addiction treatment developed by Alan Marlatt and Dennis Donovan. It is based on the understanding that relapse is a process — not an event — and that identifying warning signs early and developing effective coping strategies dramatically reduces the risk of returning to substance use.

Who Is This Therapy For?

The Scenario

A client is approaching discharge and is terrified — treatment felt safe, but the real world has their old triggers: the drive home past the liquor store, the friend who still uses, the pressure at work, the loneliness of a quiet evening.

The Speranza Approach

Together with their therapist, they map every trigger they can anticipate, write out specific coping responses, identify people they can call, and rehearse what they'll do in the highest-risk moments. The relapse prevention plan isn't a list of rules — it's a personalized operating manual they can open the moment the pressure starts to build.

How Relapse Prevention Works

Relapse Prevention begins with education: helping clients understand the Abstinence Violation Effect (AVE), the difference between a lapse and a full relapse, and the three-stage relapse process — emotional relapse, mental relapse, and physical relapse. Understanding these stages allows for much earlier intervention.

A significant portion of RP work focuses on trigger identification. Clients systematically map internal triggers (emotions, thoughts, physical states like hunger or fatigue) and external triggers (people, places, objects, situations) associated with past substance use. This map becomes the foundation of a personalized coping plan.

High-risk situations are rehearsed using cognitive and behavioral strategies. Clients practice thought-stopping, urge surfing, coping self-talk, and escape plans for situations where cravings or pressure to use emerge. Role-playing high-risk social scenarios in group settings builds confidence and preparedness.

A comprehensive written Relapse Prevention Plan is completed before each client's discharge from our programs. This plan includes triggers, warning signs, coping strategies, support contacts, self-care routines, and clear steps to take if a lapse occurs — turning the clinical work into a portable, practical guide for independent recovery.
