DBT was developed by psychologist Dr. Marsha Linehan in the late 1980s, originally for individuals with borderline personality disorder. It has since expanded into one of the most versatile and effective therapies available, now used for addiction, PTSD, depression, anxiety, eating disorders, and many other conditions.

A client has spent years at the mercy of emotional volatility — sudden anger, overwhelming urges, relationships that burn hot and cold — and feels powerless over their own reactions. Every attempt to 'just control it' has failed.

The Speranza Approach

In DBT skills groups, they learn to identify an emotion before it peaks, apply distress-tolerance techniques to survive the crisis without making it worse, and use interpersonal effectiveness skills to communicate needs without escalating conflict. The diary card becomes a mirror — they see patterns, track skill use, and gradually build genuine agency over their emotional life.

How Dialectical Behavior Therapy (DBT) Works

DBT is built on the core premise of 'dialectics' — the idea that two seemingly opposite truths can coexist. The central dialectic in DBT is acceptance and change: accepting yourself as you are in this moment while also working to change the behaviors that are harming your life.

DBT skills are taught in four modules: Mindfulness, the foundation of all DBT practice, which involves learning to observe and describe thoughts and emotions without judgment; Distress Tolerance, which teaches crisis survival skills for moments of overwhelming pain; Emotion Regulation, which develops skills to identify, understand, and change emotional responses; and Interpersonal Effectiveness, which improves communication, boundary-setting, and relationship skills.

In our programs, DBT is delivered both in individual therapy sessions and in structured DBT skills groups. The skills group format allows clients to learn, practice, and reinforce skills in a supportive peer environment.

A key feature of DBT is the emphasis on skills generalization — practicing new skills in real life situations and using diary cards to track progress, emotional patterns, and skill use between sessions.
