Treatment Should Be Accessible — Here's How

The cost of addiction and mental health treatment is one of the biggest barriers to people getting help. But if you don't have insurance, or if your insurance leaves you with a high deductible you can't afford, that doesn't mean treatment is out of reach. There are options — sliding scale fees, payment plans, state-funded programs, and nonprofit resources — that make treatment accessible.

This guide covers the options available in New Jersey for people who need treatment but are concerned about cost.

Sliding Scale Fees

A sliding scale fee is a pricing structure where the cost of treatment is based on your income and ability to pay. If you earn less, you pay less. If you earn more, you pay more. The goal is to make treatment accessible to people at all income levels.

How Sliding Scale Fees Work

Each treatment center sets its own sliding scale, but a typical structure looks like this:

  • Below 200% of the federal poverty level: Significantly reduced fees or free treatment
  • 200-300% of the federal poverty level: Reduced fees (50-75% of full cost)
  • 300-400% of the federal poverty level: Slightly reduced fees (75-90% of full cost)
  • Above 400% of the federal poverty level: Full cost

For 2026, the federal poverty level for a single person is approximately $15,650 per year. So 200% is about $31,300, and 400% is about $62,600. If you earn less than $31,300, you may qualify for significantly reduced or free treatment.

How to Access Sliding Scale Fees

  1. Ask the treatment center. When you call, ask if they offer sliding scale fees. Many centers don't advertise this, but they do offer it.
  2. Provide documentation. You'll typically need to provide proof of income — recent pay stubs, tax returns, or a letter from your employer.
  3. Negotiate. If a center doesn't have a formal sliding scale, they may still be willing to reduce fees based on your situation. It never hurts to ask.

Treatment Centers That Offer Sliding Scale Fees

In New Jersey, sliding scale fees are available at:

  • Community mental health centers (funded by the state)
  • Nonprofit treatment centers
  • Some private treatment centers (like Speranza Behavioral Health)
  • Federally Qualified Health Centers (FQHCs)

Payment Plans

A payment plan allows you to spread the cost of treatment over time, rather than paying the full amount upfront. This can make treatment accessible even if you can't afford the full cost at once.

How Payment Plans Work

Each treatment center sets its own payment plan terms, but typical structures include:

  • Monthly payments. You pay a fixed amount each month until the balance is paid off.
  • Extended terms. Plans may extend 3-12 months or longer, depending on the amount and the center's policies.
  • No interest. Many treatment centers offer interest-free payment plans, unlike credit cards or personal loans.
  • Down payment. Some centers require a down payment, with the balance spread over monthly payments.

How to Set Up a Payment Plan

  1. Ask the admissions team. When you call, ask about payment plan options. Many centers are willing to work with you.
  2. Be honest about your situation. Tell the admissions team what you can afford per month. They may be able to customize a plan that works for your budget.
  3. Get the terms in writing. Make sure you understand the monthly payment, the term length, and any fees or interest.

Using a Personal Loan or Credit Card

If a treatment center doesn't offer payment plans, or if you need more flexibility, you can use a personal loan or credit card to finance treatment. This is more expensive than an interest-free payment plan, but it can make treatment accessible.

  • Personal loans: Many lenders offer personal loans for medical expenses. Interest rates vary based on your credit.
  • Credit cards: If you have a card with a 0% introductory APR, you can finance treatment interest-free for 12-18 months.
  • Medical credit cards: CareCredit and similar cards are designed for medical expenses, including addiction treatment.

State-Funded Treatment in New Jersey

New Jersey provides state-funded treatment for people who can't afford private treatment. These programs are administered through the Division of Mental Health and Addiction Services (DMHAS).

New Jersey Division of Mental Health and Addiction Services (DMHAS)

DMHAS funds treatment throughout New Jersey, including:

How to Access State-Funded Treatment

  1. Call the NJ Addictions Services Hotline: 1-844-276-2777. This hotline can connect you with state-funded treatment in your area.
  2. Contact your county's addiction services office. Each county has an office that coordinates state-funded treatment.
  3. Be prepared for a clinical assessment. State-funded treatment requires a clinical assessment to determine the appropriate level of care.
  4. Understand the limitations. State-funded treatment may have waiting lists, less individualized care, and fewer amenities than private treatment. But it provides essential care for people who have no other options.

Medicaid

If you qualify for Medicaid (income-based, in New Jersey), Medicaid covers comprehensive addiction and mental health treatment at no cost to you. This includes detox, inpatient, PHP, IOP, outpatient therapy, and MAT.

To apply for Medicaid in New Jersey, visit nj.gov or call 1-800-701-0710.

Nonprofit and Charitable Resources

Several nonprofits provide financial assistance for addiction treatment:

SAMHSA Grants

The Substance Abuse and Mental Health Services Administration (SAMHSA) provides grants to states and organizations that fund treatment for people who can't afford it. These grants are distributed through state agencies and treatment centers.

Nonprofit Treatment Scholarships

Some treatment centers and nonprofits offer scholarships — free or reduced-cost treatment — for people who can't afford care. These are limited and competitive, but they're worth exploring.

12-Step and Mutual Aid Groups (Free)

While not a substitute for professional treatment, 12-step groups and mutual aid groups are free and can provide essential support:

  • Alcoholics Anonymous (AA)
  • Narcotics Anonymous (NA)
  • Crystal Meth Anonymous (CMA)
  • SMART Recovery (secular alternative to 12-step)
  • Dual Recovery Anonymous (for co-occurring disorders)

These groups are free, available throughout New Jersey, and can be a valuable part of a recovery plan — especially for people who can't afford professional treatment.

What to Do If You Can't Afford Treatment

If you're ready to get help but worried about cost, here's what to do:

1. Call the Treatment Center and Ask About Options

Most treatment centers, including Speranza Behavioral Health, are committed to helping people access treatment regardless of their financial situation. When you call, be honest about your financial situation. Ask about:

  • Sliding scale fees
  • Payment plans
  • Insurance verification (if you have insurance)
  • State-funded options
  • Scholarships or financial aid

2. Verify Your Insurance (If You Have It)

If you have insurance — even if you think it won't cover treatment — have it verified. You may be surprised by what's covered. Our admissions team provides free insurance verification.

3. Explore State-Funded Options

If you don't have insurance or can't afford private treatment, call the NJ Addictions Services Hotline at 1-844-276-2777 to explore state-funded options.

4. Don't Wait

The biggest mistake people make is waiting — waiting until they have more money, waiting until their insurance situation changes, waiting until they hit rock bottom. Addiction and mental health conditions get worse over time, not better. The best time to get treatment is now, and there are options to make it accessible.
